    WELCOME TO OUR 31st STATED MEETING ~ CROWNE PLAZA JACKSONVILLE AIRPORT, JACKSONVILLE, FLORIDA, SEPTEMBER 14-15, 2017, NOON TO NOON.

    COMPLETE THIS FORM FOR YOUR REGISTRATION ON OR BEFORE AUGUST 15, 2017. IF YOU ARE BRINGING A GUEST YOU MUST FILL OUT A GUEST REGISTRATION FORM FOR THEM ALSO. For a cancellation within 72 hours of the arrival date, there is no charge. If there is a cancellation after that, there is a full night's charge. IF YOU ARE UNSURE OF YOUR SCHEDULE AND NEED TO REGISTER AFTER AUGUST 15TH, PLEASE CONTACT ME, AS WE CAN REGISTER YOU CLOSER TO THE MEETING DATE. ~~~~~~~ PLEASE NOTE: TO BRING A GUEST, YOU MUST SUBMIT A GUEST REGISTRATION AND PAY THE INVOICE BY 8/20/17 (By check or money order).   • The Synod Stated Meeting will be held at CROWNE PLAZA JACKSONVILLE AIRPORT, 14670 Duval Road, Jacksonville, Florida 32218. The website is cpjacksonvilleairport.com. PLEASE CHECK ALL THAT APPLY.
  • You will be staying at THE CROWNE PLAZA JACKSONVILLE AIRPORT, JACKSONVILLE, FLORIDA; Our contact is SHAWANA SCOTT, 904.741.2032 or 904.741.4404. The Synod will pay for each person's room with a roommate. For those with a single room, the half-rate of $46.00 per night applies to each person (unless staff or EAC) and will be deducted from your travel reimbursement form, unless a medical condition or circumstance is noted. Please indicate a roommate preference; If you have none, the Synod will make arrangements for you. PLEASE NOTE: If you have an emergency and need to cancel your room, please call Crowne Plaza directly, 904.741.2032 OR 904.741.4404, then immediately notify Lisa on her cell, 540.470.0870. IF YOU ARE BRINGING A GUEST, A SINGLE ROOM RATE WILL APPLY. PLEASE CHECK ALL THAT APPLY.
  • The Synod will make arrangements for a roommate if you have no preference. Guest charges will be the cost of a single room $92.00 and approximately $175.00 in meals. IF YOU ARE BRINGING A GUEST, YOU MUST FILL OUT A GUEST REGISTRATION FORM.
  • Participants are asked to drive if they are within 300 miles, one-way of the meeting location. Airline reservations of $325 or less may be booked at any time. If over $325, please contact Lisa Lovelady to schedule a flight. Reservations should be made now to ensure best airfare. Flight arrivals to the Jacksonville International Airport must be no later than 10:30 am on Thursday. If you cannot come on Thursday by 10:30 am, please consider coming on Wednesday. Flight departures from Jacksonville International must be no earlier than 3:00 pm on Friday. Ground Transportation will be arranged by shared car rental - more detailed information to follow, as neede. CAR RENTAL MUST BE APPROVED BY THE SYNOD OFFICE. Please be sure to list your flight information on this form.

  • We have the following Group Meals planned: Wednesday Dinner for the EAC and early arrivals; Thursday Breakfast and Lunch at the Crowne Plaza, Dinner at a local restaurant, and a Friday Boxed Lunch. Meal times will be confirmed as we get closer to the date.
